CMS designed the 2004 physician payment reform to align economic incentives and improve dialysis quality. But, an analysis of the care of more than 630,000 dialyzors has found an important unintended consequence—reduced access to PD and home HD for those with traditional Medicare or those who live in areas with larger vs. smaller dialysis clinics.
