includes requirements for:
A written agreement for clinic and nursing home
responsibilities
Qualifications, training and competency testing for those who do
treatments
Coordinated communication/care planning between clinics and
nursing homes
Documentation of dialysis clinic policies and procedures
Emergency plans, including dialysis coverage and how to address

Analysis of nursing home dialysis data and how they are used for
QAPI
